To place an order, please complete the Shop OCMCA Order Form. Orders are placed on a monthly basis and OCMCA staff will contact you when your order is delivered to the OCMCA office. Order forms can be faxed or emailed to the OCMCA office. All order must be paid for upon pick up of merchandise by check (made out to Oakland County Medical Control Authority) or cash.
