EMS Provider Acknowledgement Confirmation

Thank you for submitting a request for an EMS Provider Acknowledgement. The Acknowledgement Peer Review and Recommendation Team will review all submissions and choose the event or events that are extraordinary.  The chosen agency(s) and personnel will be invited to a Medical Control Committee meeting where they will be awarded certificates and life saving pins.

All individuals that are not selected for recognition at a Medical Control Committee meeting will receive by mail, an agency letter of acknowledgement and individual award certificates from the EMS Medical Director of the OCMCA. All submissions must be received at least 30 days prior to an upcoming Medical Control Committee for eligibility.
